CASE MANAGEMENT SERVICES
H.Y.P.E. provides comprehensive case management services to ensure every youth receives the support needed to thrive. Our certified staff work collaboratively with each participant and their caregivers to coordinate access to behavioral health care, educational resources, social supports, and community services. Through consistent check-ins, advocacy, and connection to culturally relevant resources, our team helps reduce barriers, build stability, and promote long-term success.
Group Services
Our group-based sessions are designed to create a safe, interactive space where youth can build critical life skills, develop emotional regulation techniques, and strengthen their social connections. Led by trained facilitators, these sessions focus on topics like coping strategies, peer relationships, anger management, and self-esteem. Group services encourage shared learning, peer accountability, and a sense of belonging—all essential for personal growth and community wellness.
Family & Individual Counseling
H.Y.P.E. offers trauma-aware counseling services tailored to the unique experiences of each youth and their families. Whether through one-on-one sessions or family-based work, our licensed professionals help participants explore emotions, navigate conflict, and build stronger, healthier relationships. Counseling supports the development of insight, communication skills, and coping strategies—empowering families to work together toward shared healing and stability.
Individualized Treatment Plan
Each youth enrolled in H.Y.P.E. receives a customized treatment plan, developed in collaboration with their care team and family. Grounded in a strengths-based approach, the plan outlines personalized goals, therapeutic interventions, and measurable outcomes aligned with the youth’s developmental needs. Plans are regularly reviewed and updated to reflect progress and evolving priorities, ensuring every participant stays on track for success.
Diagnostic Assessments
Our intake process begins with a thorough diagnostic assessment, conducted by a qualified clinician to identify the specific behavioral, emotional, and developmental needs of each youth. This assessment guides the selection of services, informs the treatment plan, and ensures that supports are appropriate, targeted, and effective. The process includes clinical interviews, behavioral observations, and when appropriate, caregiver input, to provide a full picture of the youth’s wellness needs.
